Housing Search Specialist – Job Opening!

Housing Search Entity Specialist

Full time position (35 hours) to assist individuals enrolled in the ABI-N or MFP-CL Waivers transitioning from a skilled facility setting to housing within the community by:

  • Helping to identify appropriate housing opportunities that meet or can be reasonably modified to meet their individualized needs and preferences, as documented by their Case Manager;
  • Planning a complete and time-sensitive housing search;
  • Matching an enrollee with applicable preferences for subsidized housing;
  • Securing application materials for housing options preferred by each referred individual;
  • Reviewing applications to ensure accurate completion and inclusion of required verification documentation;
  • Tracking housing application receipts, updates, and waiting list status;
  • Addressing obstacles that arise to housing access such as the need for reasonable accommodation requests, and education needs of a housing owner/agent;
  • Coordinating with each referred individual and their case managers to view and assess units, and meet with potential landlords;
  • Assisting these individuals to follow up on status determinations; and
  • Ensuring that a lease is utilized by the landlord.

Position: Registered Nurse

Full time Registered Nurse needed to complete in home evaluations in Hampden County and surrounding area for state funded program. Requires at least 1 year community health experience, knowledge of disabilities and community resources. Proficiency with medical documentation and reliable transportation required. Bilingual preferred.
