Youth Services Peer/Advocate Counselor– Job Opening!

Private nonprofit disability organization seeks full-time individual to work with youth with disabilities.  Applicant is expected to provide independent living skills training,  peer counseling, transportation orientation, and teach self-advocacy skills to transition age youth. Must have personal experience with disabilities. Group facilitation skills and individual advocacy experience desirable.  Advocate will be based out of the Amherst office. Some evenings required. Position: Information and Referral Spacialist

Looking for motivated bilingual individual (English/Spanish) to join our Information and Referral team. This is a full-time (35 hours per week) position based in the Amherst office. Individual is expected to respond to requests for general information about Stavros services and other agencies and services benefiting people with disabilities.

Responsibilities include, but are not limited to:

Managing high call volume in a timely manner.

Directing callers to the resources available both internally and externally in a professional manner.

Good communication skill to ensure respectful delivery of service.

Working as a team with staff to ensure the quality of services delivered.

Remain current with community resources assisting people with disabilities.

Attend all required trainings.

Ability to accurately record all information and appropriate referrals in our computerized systems.

Full time Registered Nurse needed to complete in home evaluations in Hampden County and surrounding area for state funded program. Requires at least 1 year community health experience, knowledge of disabilities and community resources. Proficiency with medical documentation and reliable transportation required. Position: Skills Trainer

Description:  Skills Trainer out of Amherst office. See below. Seeking individual with excellent communication and organizational skills. This position is to educate people with disabilities about available community programs and services. Must be self- motivated, able to work independently; have reliable transportation and willing to travel. Understanding of PCA program will be useful. It is a full time position with excellent benefits. 

Skill Trainer will provide the following services, for MassHealth members who apply to Stavros for the PCA Services:  Assessment, Orientation and Skills Training.  To educate and assist individuals with disabilities to acquire and maintain personal assistant services so that they will be able to effectively manage their own PCA programs and live independently. In addition, to monitor the progress in acquiring and maintaining personal care assistant services.

Requirements: To remain informed about State and Federal regulatory changes in policy and procedures as they are related to the Personal Assistant Services through readings and meetings assigned by the Director of Skills Trainers and Skills Trainer Coordinator. To review the performance of PCA consumer and/or surrogates to ensure that they are administering the Program in compliance with the MassHealth PCA program regulations and ensuring that the participant has maximum control over their programs. To identify additional needs of Consumer that help maximizes their independents; including but not limited to transportation, housing, communication needs, social/recreational, advocacy, assistance equipment, and MassHealth (Medicaid) eligibility and to report these to the Skills Trainer Coordinator. To be knowledgeable about the services provided by Stavros; to represent Stavros in a professional manner; to be aware of the independent living philosophy and encourage and foster others; to take/keep control of their lives.

Must have personal experience with disabilities, be well-organized, computer literate, and have excellent written/verbal communication skills. Reliable transportation a must. Bi-lingual applicants encouraged to apply. People with disabilities encouraged to apply. AA/EOE.
