Contracted Services: RN or LPN for Home Evaluations
Contract Nurse: Variable Hours / Not a benefits eligible position Are you currently a Registered Nurse or LPN that enjoys working in the Community/ Home Health Care niche of the highly skilled area of Nursing? Stavros is one of the oldest Centers for Independent Living in the country with a mission-based philosophy of providing services and supports to the community it serves for nearly 50 years! The nurses of the PCM department provide the integral service of carrying out the initial evaluations and annual re-evaluations critical to allowing the necessary services to begin, continue, evolve etc.

Long Term Service Coordinator
Description
Stavros is actively seeking to fulfill an opening within the One Care Department to help coordinate needed services for people with disabilities who live at home. The primary responsibilities would include person centered care planning, coordination of services and supports that prevents people from going into a nursing facility. This position will also serve as an advocate for consumers needing services to meet their needs.

Skills Trainer
(This is a Full time 35 hours / Week Benefits Eligible Position)
The Skills Trainer provides and monitors services for Consumers utilizing the MassHealth Personal Care Assistant (PCA) Program with duties including but not limited to: Intake and Orientation to the program, Assessments and Skills Training all with the goal of educating and assisting individuals with disabilities in order to acquire and maintain PCA services so that they will be able to effectively manage the program and live independently.
