Long Term Service & Supports Coordinator
(Spanish / English) (This is a Full time 35 hours / Week Benefits Eligible Position)
Stavros is seeking to fulfill the position of Long Term Service and Supports Coordinator to provide long-term services and supports (LTSS) planning, coordination, advocacy to members of One Care Program. The independence and well-being of People with Disabilities depends on multiple aspects, in large part by those committed to roles including the Long Term Services and Supports Coordinator (LTSS Coordinator). Stavros has been a place of support, advocacy and service coordination for the community it serves for over 50 years.
Stavros is actively seeking to fill an opening within the One Care Department to serve as an LTSS Coordinator whose primary responsibilities would include person centered care planning, coordination of long term services and supports, and advocacy to members of the One Care Program

Advocate / Peer Counselor
(This is a Full time 35 hours / Week Benefits Eligible Position)
A great fit for this position is an energetic person with a passion for helping people with disabilities; one who is willing to work with persons with disabilities to identify needed resources and training to make a difference in people's lives by providing and/or coordinating independent living life skills training, peer counseling, and advocacy with and for individuals toward the achievement of their personal goals.

Staff Nurses; In-Home
(This is a Full time 35 hours / Week Benefits Eligible Position)
Stavros is seeking to hire Staff Nurses (RN’s/LPN’s) who would be meeting People with Disabilities (aka Consumers) in their homes, located in the Hampden, Hampshire and Franklin counties to complete initial and annual In-Home Evaluations and Re-Evaluations. These evaluations are essential in identifying the available resources and supports needed by the consumers.

Skills Trainer

Member Services
(This is a Full time 35 hours / Week Benefits Eligible Position)
We are seeking a customer service representative for our Member Services position who can provide in-person and telephone support to people with disabilities inquiring about Personal Care Services concerns. Other job responsibilities include assisting walk-in and on-the-phone consumers, taking and directing calls, documenting calls and complaints, documenting and processing any appeal requests for denial of services, providing administrative support to the department, and verifying MassHealth eligibility, updating consumers’ personal information.
